Scenario You should use your own work practice/placement from the Health & Social Care sector to support your answers for ALL tasks (or scenario/ case study). Case Study A summary of the serious case review of Adult A who had a moderate learning disability and who died suddenly on 10 March 2010 whilst clearing papers young people had scattered in his garden. His death was found to be from natural causes (the combination of a heart attack and a small cancerous tumour), but the pathologist stated that the stress of the harassment experienced by Adult A and his family during the day and evening of his death could have been exacerbated the medical issues. The family suffered many incidents of harassment since moving to their house in 1971. During the period from January 2007 to March 2010 over 90 recorded incidents of burglary, harassment and general anti-social incidents were recorded by a variety of agencies. Many incidents had been reported to the police. The review concluded that although agencies had taken, there were often delays and action was uncoordinated. A number of recommendations were made, including the need for improved information and sharing between agencies.
