Robert Wood Johnson Foundation Committee | Iom Future Of NursingRobert Wood Johnson Foundation Committee  was passed by the Congress, called the Affordable Care Act, which got sanctioned by the president to be implemented for a better and transformed health care system in United Stated of America. The motive behind this is to make it more affordable to all the people irrespective of where they live and their standard of living. The Robert Wood Johnson Foundation Committee collaborated with the Institute of Medicine (IOM) to envision a transformation required in the nursing field, keeping in mind the importance of this profession in order to meet the medical needs of the people and for the people to employ the privileges provided by the ‘Affordable Care Act’.  The Future of Nursing: Leading Change, Advancing Health is the report prepared by the Institute of Medicine which examines the nursing workforce thoroughly and presented recommendations that would focus on the actions that must taken to improve the nursing workforce and how it can be utilised in order to meet the needs of the patients. Currently, nurses hold different types of degrees in accordance to which they work in different settings. On one hand, some of them are scientists who contribute to the ongoing research on how to care for a patient more effectively, whereas on the other some are licensed nurses working in nursing homes and hospitals and hence are more closely related and more responsible for the health care of a patient. The four main issues that were raised in the recommendation report offered by the Institute of Medicine are:

  1. Nurses should practice to the full extent of their education and training.
  2. Nurses should achieve higher levels of education and training through an improved education system that promoted seamless academic progression.
  3. Nurses should be full partners, with physicians and other health professionals, in redesigning health care in the United States.
  4. Effective workforce planning and policy making require better data collection and information infrastructure.

This report is to be considered as a blueprints focusing on implementing diversity in the nursing profession and the necessary actions that must be taken by the nursing workforce to meet the health care needs of the diverse population of United States.  Like discussed earlier nurses hold different degree of qualifications, some are registered nurses (RNs) whereas a quarter of a million nurses are advance practice registered nurses (APRN), who have passed national certification exams holding master’s and doctoral degrees. Now, the rules and regulation of practicing and licensing are different in each state. Hence if one state allows the nurses to prescribe medicines to the patients without the supervision of any doctor, the other on the contrary do not exercise any such rule which limits the actions that can be taken by a nurse for the benefit of a patient. We can see that it’s not the education of the nurse but the state laws that determine the actions of a nurse. According to this report the nurses should have the liberty to implement their education to the full extent. The federal government has the authority to amend a law practiced in a state in order put it in better practice for the advantage of the population of that state. The report also suggests for a better residency programme for nurses, which would increase the chances of new graduates to practice and improve their practice and skills.

For providing better and higher quality of health care and medical attention it must be necessary for the nurses to expand their boundaries of knowledge and attain higher levels of education. This would require the licensed practical as well as vocational nurses with diplomas to further educate themselves in attaining associate’s degree, bachelor’s degree, master’s degree, doctoral degree or doctor of nursing practice degrees. In addition to that they should be continuously trained and educated along with doctors and other healthcare professionals. Nurses should be competent in areas like leadership, health care policy, teamwork etc. and should also be well aware about community and public health and geriatrics. Promoting diversity of race ethnicity and gender in the nursing workforce is also a major factor in a better health care system.

It is extremely important to encourage leadership among nurses so that they can collaborate with doctors and other health care professionals. They must be prepared to advance in their profession from all aspects like high quality care, improve and reform plans in order to achieve the required goals, taking responsibility of the patient and take actions in accordance to the need of the patient. In order to promote these qualities among nurses the education programmers must implement tasks that will help them to achieve leadership competencies. These areas of training must also be made available to nurses at all levels. Nurses must imbibe these qualities to elevate their personal and professional growth.

In order to implement these changes a primary planning is required to evaluate the current healthcare infrastructure and the types and numbers of professionals related to heath care including the nurses. As soon as the improved infrastructure comes into action a planned systematic assessment, based on demographics, skill mix, region, role etc, will apply the changes that must be enforced according to the workforce requirement for a better nursing education and practice. Both National Health Care Workforce and National Centre for Workforce Analysis were set up as authorised by the ‘Affordable Care Act’ 2010. These programmes will focus on evaluating data and supervise the supply of health care workers across profession. These will further predict the workforce requirements, based on their evaluation, and coordinate with the regional and state health care workforce and units to bring about the necessary changes. All of these data will be publicly accessible.

Improvements in the profession of nursing play a fundamental role if the people of United States are to enjoy the privileges of the ‘Affordable Care Act’. However in the smooth working of a business and organisations government, health care organisation, insurance industry, professional associations, also play an important role.

Future of Nursing: Campaign for Action has four main strategies that they are working on. They are:

  1. Collaborate with a broad array of stakeholders.
  2. Activate on local, state and national levels.
  3. Communicate the call to action.
  4. Monitor results to ensure accountability.

State action coalition is working like a driving force in order to implement the recommendations report by the Institute of Medicine in every state. It has formed a network promoting of Nursing: Campaign for Action.
